Your privacy is of the utmost importance to us. UF Health will use your confidential medical information in order to provide you health care services. UF Health will, at all times, maintain your confidential medical information in strict confidence and will not disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law. Please review the UF Health Joint Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyUFHealth that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.
MyUFHealth should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is 2 business days. Therefore, for all urgent medical matters, please contact your physician’s office by phone, go to an emergency room, or dial 911.If your provider is out of the office or unavailable, messages sent via MyUFHealth may be routed to other authorized caregivers within UF Health in order to facilitate a timely response to your request.
All communications between you and UF Health using MyUFHealth occur over a secure connection. You will receive Internet e-mail messages notifying you that new information is available in your MyUFHealth account. These e-mail messages, however, will not contain any confidential medical information. All of the confidential medical information available to you in MyUFHealth is protected and securely maintained by UF Health.
When you initially enroll in MyUFHealth, you will be provided with an access code. The access code you receive will give you the opportunity to create a personal login name (called “MyUFHealth ID”) and a password. Once this access code has been used it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if they were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that your security has been compromised. You may change your password at any time.
It is very important that you let us know as far in advance as possible when you need to cancel your appointment. This allows us to offer that appointment slot to another patient. If you do not cancel your appointment and you fail to keep it you are considered a no show. Excessive no shows may result in your being discharged from your provider’s office. In addition, directly scheduling appointments in MyUFHealth that are not needed or are frequently canceled, may result in the termination of your MyUFHealth account.
Information Does Not Constitute Medical Advice
UF Health assumes no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you.
You may be accessing your own medical information via the MyUFHealth application and may wish to communicate with one of your health care providers about that or related information. If you have an emergency, or think you need to speak to someone urgently, please do not use the email communication options provided to you by MyUFHealth. Neither MyUFHealth nor any other application available through this Site is a substitute for appropriate and timely contact with your physician. This Site is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.
Termination of Access
UF Health may restrict or terminate access to MyUFHealth at any time. If access is restricted for planned maintenance of the MyUFHealth system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyUFHealth without advance notice. If your use of MyUFHealth violates these Terms and Conditions or is inappropriate as judged in the sole discretion of UF Health, we reserve the right to permanently terminate your access to MyUFHealth.
Restrictions on Use and Ownership
All pages within this Site and any material made available for download are the property of UF Health and/or its subsidiaries and affiliates. The content on the Site is provided for UF Health and its patients only. The words “University of Florida,” “UF,” “UF Physicians,” “UF Health,” and its associated corporate logo are the trademarks and service marks of University of Florida Board of Trustees. Modification of MyUFHealth
UF Health reserves the right at any time and from time to time to modify or discontinue, temporarily or permanently, MyUFHealth (or any part thereof). You agree that UF Health shall not be liable to you or to any third party for any modification, suspension, or discontinuance of MyUFHealth. MyUFHealth may specify from time to time the version(s) of related products required in order to use MyUFHealth (e.g. supported browsers).
Choice of Law
You agree that with respect to any suit, action or proceeding arising out of or in connection with any dispute with us related to MyUFHealth, (i) you will submit to the state or federal courts of the State of Florida, and (ii) you irrevocably waive any objection that you may have at any time to the laying of venue of any suit, action or proceeding arising out of or relating hereto brought in any such court; irrevocably waive any claim that any such suit, action or proceeding brought in any such court has been brought in an inconvenient forum; and irrevocably waive the right to object, with respect to any such claim, suit, action or proceeding brought in any such court, that such court does not have jurisdiction over you.
*For purposes of this agreement, UF Health describes a collaboration of the University of Florida Board of Trustees for the benefit of the University of Florida College of Medicine, Shands Jacksonville Medical Center, Inc., Shands Teaching Hospital and Clinics, Inc., and Shands Recovery, LLC. Collectively, these entities are referred to as UF Health in this form.
